What do I do if I have forgotten my username and/or password?

Solution

Please submit a help desk ticket. In addition to providing the information in the required fields (indicated by asterisks --"*"), please provide your full name so that we can locate your account.

Once we receive your ticket, our help desk staff should be able to respond with your login information (usually within 12 to 24 hours).
